Part I
First, we need to connect SurveyMonkey and Salesforce to Zapier.
Create Zapier account.

Set up a Trigger action for SurveyMonkey
Choose the following trigger: New Response Notification With Answers (Triggers a notification when your survey is completed and sends the entire response. Only for SurveyMonkey annual paid users.).
Then set up an Action for Salesforce
Choose this action: Create Record (Creates a new record of a specified Salesforce object (ie. Contact, Lead, Opportunity, etc.).).

Finally, we need to test this automation and publish it.

Configure the data that is exchanged between SurveyMonkey and Salesforce.
Test the workflow and turn it on for production use
Test your scenario and publish to production. Now you have an automatic workflow, that will Add new SurveyMonkey responses to Salesforce as notes!
